How to Enroll in a Pooled Trust

To enroll in a pooled trust with NYSARC Trust Services, simply complete the Joinder Agreement (trust application), attach the necessary supporting documentation, and mail the original copies to NYSARC Trust Services with a minimum initial deposit of $300.

Step 1: Gather Necessary Documentation

Here are the items currently required when submitting an application:

Depending on your situation, you may also need to provide:

  • Copy of POA or Guardianship, if Joinder Agreement is signed by agent or guardian
  • If account is established by Court Order, a copy of the Court Order is required

Step 2: Complete Application (Joinder Agreement)

  • Complete the Joinder Agreement and have it notarized
  • The application for Community Trust I and Community Trust II are identical 
  • Can be signed by the beneficiary, Power of Attorney (POA), guardian, parent or grandparent
    • If signed by POA - submit a copy of the POA
    • If signed by Guardian - submit court decree

Step 3: Mail Original to NYSARC Trust Services

  • Mail the original, notarized Joinder Agreement with supporting documentation and initial funding to NYSARC Trust Services:

  • Initial Deposit Amount:
    • There is a $200 one-time enrollment fee
    • You must open the account with a minimum of $300 in guaranteed funds (e.g. cashier's check, money order, or certified check) made payable to NYSARC, Inc. Community Trust
    • You may also open a trust with your initial deposit of excess income or resources
