Third Party Community Trust Documents

Thank you for your interest in establishing a Third Party Community Trust to protect your loved one’s benefit eligibility. To get started, the Donor must complete the Joinder Agreement to establish an account during their lifetime. See Instructions to Establish an Account for more information.

The Donor will need to meet with an attorney to create or update your family’s estate plan, wills, titling of assets, and Beneficiary designations on insurance policies and retirement plans. Attorneys may contact our office to request the appropriate Beneficiary Designation language.
